{"id":12410,"date":"2011-06-28T10:54:37","date_gmt":"2011-06-28T14:54:37","guid":{"rendered":"http:\/\/researchimpact.ydsinc.ca\/small-steps-towards-a-big-problem\/"},"modified":"2021-11-23T15:55:32","modified_gmt":"2021-11-23T20:55:32","slug":"small-steps-towards-a-big-problem","status":"publish","type":"post","link":"https:\/\/researchimpact.ca\/fr\/archive\/small-steps-towards-a-big-problem\/","title":{"rendered":"Small steps towards a big problem: Addressing the social determinants of health at the community level"},"content":{"rendered":"<p><em>David Phipps (ResearchImpact, York) wrote the following guest blog post for \u00ab\u00a0<a href=\"http:\/\/www.healthypolicies.com\/\">Health Policies<\/a>\u00a0\u00bb <em>about a new initiative with UWYR that is focused on community solutions for community health challenges arising from the social determinants of health<\/em><em>.<\/em><br \/>\n<\/em><br \/>\nHealth isn\u2019t a problem.\u00a0 Not being healthy is a <a href=\"http:\/\/en.wikipedia.org\/wiki\/Wicked_problem\">wicked problem<\/a>.\u00a0 Wicked problems are persistent social problems characterized (among other things) by:<\/p>\n<ul>\n<li>Lack of clarity on all stakeholders associated with the problem<\/li>\n<li>Lack of clarity on the causes of the problems<\/li>\n<li>Lack of clarity on end points and outcomes<\/li>\n<li>Interventions change the nature of the wicked problem challenging evaluation<\/li>\n<\/ul>\n<p>Social determinants of health (SDOH) are wicked problems.<br \/>\nThere is an increasing amount of attention paid to SDOH at the international, national and local levels:<\/p>\n<ul>\n<li>International: The World Health Organization recently released the <a href=\"http:\/\/www.who.int\/sdhconference\/consultation\/draft_WCSDH_Technical_Paper.pdf\">technical paper<\/a> for the World Conference on Social Determinants of Health to be held in Rio de Janeiro in October 2011.<\/li>\n<li>National: National Collaborating Centre for Determinants of Health (NCCDH) is one of six NCCs funded by the Public Health Agency of Canada (PHAC). In 2010 it released an <a href=\"http:\/\/www.nccdh.ca\/supportfiles\/Environmental-Scan-2010.pdf\">environmental scan<\/a> of the role of public health agencies in supporting policy responses to social determinants of health in Canada.<\/li>\n<li>Local: On June 22 York Region\u2019s Human Services Planning Board release their report, <em>Making Ends Meet<\/em> which identifies poverty and income insecurity as the single human service priority for York Region.\u00a0 Health indicators and outcomes are included in the planning.<\/li>\n<\/ul>\n<p>But how do you tackle such large, wicked problems.\u00a0 According to WHO <a href=\"http:\/\/whqlibdoc.who.int\/publications\/2008\/9789241563703_eng.pdf\">Commission on Social Determinants of Health<\/a>, all you need to do is:<a href=\"http:\/\/www.healthypolicies.com\/wp-content\/uploads\/2011\/06\/who-logo-en.jpg\"><img loading=\"lazy\" decoding=\"async\" class=\"alignright\" title=\"who-logo-en\" src=\"http:\/\/www.healthypolicies.com\/wp-content\/uploads\/2011\/06\/who-logo-en.jpg\" alt=\"\" width=\"250\" height=\"90\" \/><\/a><\/p>\n<ol>\n<li>Improve living conditions<\/li>\n<li>Tackle inequitable distribution of power, money and resources<\/li>\n<li>Measure and understand the problem and assess the impact of action.<\/li>\n<\/ol>\n<p>I\u2019ll get right on that. I\u2019ll also get right on solving the upstream political issues underlying SDOH that were pointed out in a previous <em>Healthy Policies<\/em> <a href=\"http:\/\/www.healthypolicies.com\/2011\/02\/social-determinants-of-health-and-political-responsibility\/\">blog<\/a>.<br \/>\n<strong>NOT<\/strong> (and therein lies the problem \u2013 where do you start with a wicked problem like SDOH?)<!--more--><br \/>\nIn York Region we are taking small steps to address this wicked problem.\u00a0 At our Knowledge Mobilization Expo on June 15 we <a href=\"..\/2011\/06\/15\/kmb-press-release-mdc-communique-de-presse\/\">announced<\/a> a small grant of $93,000 from the Canadian Institutes of Health Research to take our expertise in <a href=\"http:\/\/www.healthypolicies.com\/wp-content\/uploads\/2011\/06\/YorkULogoVerlarge-e1309188359424.gif\"><img loading=\"lazy\" decoding=\"async\" class=\"alignleft\" title=\"YorkULogoVer(large)\" src=\"http:\/\/www.healthypolicies.com\/wp-content\/uploads\/2011\/06\/YorkULogoVerlarge-e1309188359424.gif\" alt=\"\" width=\"100\" height=\"142\" \/><\/a>knowledge mobilization and focus it on community solutions to community health challenges. Using <em>Making Ends <\/em><em>Meet<\/em> as a call to action, we shall hire a knowledge broker to work in the community.\u00a0 Based at UWYR the knowledge broker will work with citizens, citizen groups and agencies to identify community health challenges (all of which will be SDOH). S\/he will then broker relationships<a href=\"http:\/\/www.healthypolicies.com\/wp-content\/uploads\/2011\/06\/UYWR-logo-e1309188249540.jpg\"><img loading=\"lazy\" decoding=\"async\" class=\"alignright\" title=\"UYWR logo\" src=\"http:\/\/www.healthypolicies.com\/wp-content\/uploads\/2011\/06\/UYWR-logo-e1309188249540.jpg\" alt=\"\" width=\"100\" height=\"75\" \/><\/a> with community health providers and researchers\/grad students from York University to co-create social innovations aimed at improving the health of York Region citizens.\u00a0 One neighbourhood at a time. In this partnership the university is in a supporting role, supporting community solutions to community health challenges.\u00a0 This helps us address some of the needs identified at a recent <a href=\"..\/2011\/05\/03\/what-determines-your-health-quest-ce-qui-determine-votre-sante\/\">community\/university forum<\/a> on SDOH.<br \/>\nOne such innovation we supported previously was the heat registry of the Parkdale Activity and Recreation Centre. The <a href=\"http:\/\/www.yorku.ca\/yfile\/archive\/index.asp?Article=10875\">PARC Heat Registry<\/a> received continuing funding and was able to renew Canada\u2019s first heat registry thanks, in part, to evidence collected by a York University Knowledge Moilization Intern. The heat registry tracks and provides services to poor and vulnerable populations at risk of heat exposure on hot summer days. The heat registry has now been emulated by Toronto Public Health and has been expanded across Toronto\u2019s most at risk communities.<br \/>\nInnovations such as this will not address the underlying causes of poverty and social exclusion but the innovations will allow communities to develop health policies and health services that address those local health inequities that arise from them. They are band aids.\u00a0 We will not be raising people out of poverty but we will be addressing the health needs arising from poverty.<br \/>\nPoverty and the health inequities that arise from poverty are wicked problems.\u00a0 In York Region we are harnessing the knowledge and experience of the community, linking it to knowledge and experience in the university (=knowledge mobilization) and developing community solutions (=social innovations) to community health challenges.<br \/>\nThey are small steps but if you\u2019re the one in the community taking them, they are important to you.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>David Phipps (ResearchImpact, York) wrote the following guest blog post for \u00ab\u00a0Health Policies\u00a0\u00bb about a new initiative with UWYR that is focused on community solutions for community health challenges arising from the social determinants of health.
